A rate-and-term refinance replaces the first lien to change rate or term. Cash-out replaces the first lien and puts cash in your pocket. A HELOC sits in second position and lets you draw as needed.

When a refinance is worth it
Closing costs have to be earned back. If you plan to sell soon, a small rate cut may not pay for itself. If you need cash for a tax event, a renovation, or a down payment on another property, cash-out or a HELOC can beat stretching a credit card.

Conventional, FHA, and VA refinance
Conventional rate-and-term or cash-out when the file still looks agency. FHA streamline when you are already FHA and the math works. VA IRRRL or VA cash-out when entitlement and occupancy fit.
Residual income still applies on VA.
HELOC vs cash-out vs Equity Access
A cash-out refinance makes sense when replacing the first lien also improves the rate or term. A HELOC makes sense when you want a revolving line and the first lien is already fine. Equity Access is a third path when occupancy and income caps fit.
